Coffey Health System

The Coffey Health Foundation is a 501(c)3 charitable organization that supports the important work of Coffey Health System and invests in the future of our community.

This directory is a result of the Community Health Needs Assessment, a partnership between the Coffey County Health Department and Coffey Health System.

A Community Health Needs Assessment is a component of the Affordable Care Act established to promote a proactive stance regarding population health.

First Lt. William Fitzsimons is honored with a mural on our CHS Rehabilitation and Wellness facility (409 Cross St. in Burlington). Born in Burlington on April 18, 1889, he grew up here and became a surgeon. He was among the first to volunteer when hostilities broke out in Europe. On September 4, 1917, First Lt. William T. Fitzsimons entered U.S. history as the first American officer killed in World War I.

A Patient and Family Advisory Council (PFAC) joins patients and families with members of the healthcare team to provide guidance on how to improve the patient and family experience.
